Lactobacillus plantarum 299v is generally well tolerated in healthy adults and children. Some people taking it alongside other probiotics have reported mild gastrointestinal discomfort — bloating, gas, abdominal pain, or diarrhea — though these events are uncommon.

In rare cases, the live bacteria have been associated with serious infections in people with compromised immune systems; most of those cases involved bacteria that had moved from other parts of the body. Since data are limited, check with your doctor before starting.

L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
