Pycnogenol is generally well tolerated in studies, but long-term safety hasn't been fully nailed down. The most common side effects are gastrointestinal complaints, dizziness, and vertigo.

Headache has been reported in a trial, and there's one report of chest pain and one of heart failure worsening in patients taking the extract — though in both cases the patients had existing heart disease, and it's unclear if the extract was the cause. During pregnancy and breastfeeding, there isn't enough safety data.

Theoretically, maritime pine bark extract might increase the risk of bleeding when used with antiplatelet or anticoagulant drugs.
Clinical research suggests that maritime pine bark extract inhibits platelet aggregation. However, the clinical significance of this effect is unclear.
