Rosemary extract in normal culinary amounts is generally well tolerated; concentrated extracts need more caution. In sensitive people, it can cause allergic reactions, lip or gum swelling, and allergic skin reactions when applied to the skin.

Occupational asthma and photosensitivity (increased sun sensitivity) have been reported. The undiluted oil may cause seizures.

In pregnancy, food amounts are likely okay, but medicinal doses should be avoided; in breastfeeding, there isn't enough safety data for medicinal doses, so avoid them. Banaba leaf extract is generally well tolerated in short-term studies, but long-term safety isn't well established.

Animal research shows that rosemary extract can decrease blood glucose levels in diabetic models. However, research in humans is conflicting. Although rosemary powder decreased blood glucose levels in healthy adults, no change in blood glucose levels was seen in adults with type 2 diabetes, most of whom were taking antidiabetes drugs.
