Salacia is generally well tolerated in the short term, but long-term safety hasn't been well studied. The most common side effects are gastrointestinal — abdominal pain, diarrhea, indigestion, flatulence, and nausea — and these tend to be dose-dependent.

These happen because your bowel ferments undigested carbohydrates when salacia slows their absorption. Salacia should not be used during pregnancy; there isn't enough safety data.
