HelloPharmacist Scorecard of Sang Bai Pi by Hawaii Pharm
Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
Sang Bai Pi contains one active ingredient: certified organic white mulberry leaf extract. White mulberry leaf has been studied for its effects on blood sugar, cholesterol, bone health, and immune function, though the evidence for most of these uses remains limited.
The product also contains vegetable palm glycerin and pharmaceutical grade alcohol as inactive ingredients to preserve the liquid extract.
Does it work?
Couldn't assess
The evidence for white mulberry leaf's effectiveness isn't established in our data. The product has been studied for androgenic alopecia (pattern hair loss), osteoporosis, common cold, diabetes, and high cholesterol (hyperlipidemia), but all of these conditions carry a rating of insufficient reliable evidence—meaning we don't yet have enough quality research to know whether it actually works for them.
How safe is it?
Well-documented data
White mulberry leaf is generally well tolerated when taken by mouth. In clinical studies, some people experienced bloating, flatulence, loose stools, or constipation—but these side effects often lessened over time and were transient.
You should know that concentrated supplements are less studied than the food form, and they do lower blood sugar. There isn't enough safety data to recommend white mulberry leaf in medicinal amounts during pregnancy or while breastfeeding.
Talk with your doctor or pharmacist before taking it if you're pregnant, nursing, or planning to become pregnant.
Meds to double-check
Moderate interaction found
Before taking Sang Bai Pi, check with your doctor or pharmacist if you take antidiabetes drugs (there's a Moderate risk of low blood sugar) or medications that are OCT2 substrates cleared by your kidneys (there's a Moderate risk that this product could slow their elimination and increase side effects). Metformin is a common example of the second group.
The bottom line
Scorecard at a glanceFully disclosed formula with no assessable stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
Sang Bai Pi might interest someone looking to support blood sugar balance, though the evidence for its effectiveness isn't yet established. If you take antidiabetes medications or drugs that your kidneys clear, you should check your exact medications with your doctor or pharmacist before starting.
Digestive side effects like bloating or loose stools can happen but often fade, and you'll want to monitor your blood sugar if you're on diabetes medication.

Sang Bai Pi by Hawaii Pharm Drug Interactions
HelloPharmacist Interaction Report
Sang Bai Pi by Hawaii Pharm, which contains white mulberry leaf extract, does interact with medications—specifically through effects on blood sugar control and how your kidneys clear certain drugs.
The most serious interaction is Moderate in severity: white mulberry leaf can lower blood glucose levels, which means it might increase the risk of low blood sugar (hypoglycemia) when you're also taking antidiabetes drugs. Clinical research supports this effect, so you'll want to monitor your blood sugar closely if you're on diabetes medication.
Read the full breakdown — every affected drug type, severity by severity
White mulberry leaf also has a Moderate interaction with OCT2 substrates—a group of drugs that your kidneys filter out using a specific transport system. Animal studies show that white mulberry leaf can slow how fast your kidneys clear these drugs, potentially raising their levels in your body and increasing side effects.
Metformin, a common diabetes medication, is one example.
Altogether, these interactions span 87 individual medications. Use the interaction checker below to see if any of your specific prescriptions are affected, and talk with your doctor or pharmacist before starting this product if you take antidiabetes drugs or any medication cleared by your kidneys.

Antidiabetes Drugs
Theoretically, white mulberry leaf might increase the risk of hypoglycemia when taken with antidiabetes drugs.
Clinical research shows that white mulberry leaf can lower blood glucose levels. Advise patients to monitor glucose levels closely.
Organic Cation Transporter 2 (Oct2) Substrates
Theoretically, white mulberry leaf might slow the elimination and increase the adverse effects of drugs which are OCT2 substrates.
Animal research shows that coadministration of white mulberry leaf extract with metformin, an OCT2 substrate, slows the renal elimination of metformin via inhibition of OCT2 activity. OCT2 is expressed in the kidneys and is responsible for transporting cationic drugs into tubular epithelial cells in order to be excreted in the urine.
