Saw palmetto is generally well tolerated, and any side effects tend to be mild and reversible. In trials, the most common ones reported were abdominal pain, constipation, decreased libido, diarrhea, dizziness, fatigue, headache, nausea, rhinitis, and vomiting.

Rarely, cases of heart palpitations, changes in heart rhythm, high or low blood pressure, and one report of severe slowed heart rate have been described. A few isolated cases of skin reactions and worsening acne have also been reported.
