Saw palmetto is generally well tolerated in adults, and most side effects are mild and go away on their own. The most common ones reported are abdominal pain, constipation, diarrhea, dizziness, headache, nausea, and fatigue.

A small number of people have had decreases in libido or developed a skin rash (fixed drug eruption) after starting it. Rarely, people taking saw palmetto have reported high blood pressure, low blood pressure when standing up, irregular heartbeat, or chest pain, though these are uncommon.
