Saw palmetto is generally well tolerated, and any side effects that do occur tend to be mild and reversible. The most common ones are abdominal pain, constipation, diarrhea, nausea, dizziness, headache, fatigue, decreased sexual desire, vomiting, and stuffiness.

Rarely, cases of skin reactions (including a fixed drug eruption with blisters), worsening acne (when combined with beta-sitosterol), and heart rhythm changes have been reported. The safety data advises against use during pregnancy because of possible hormonal effects and lack of safety studies.
