When you eat shiitake as a cooked food, it's generally well tolerated. Concentrated supplements are a different story — there isn't enough safety data on supplement doses to say how safe they are, so caution is advised.

The most common side effects from the supplement form are abdominal discomfort, bloating, diarrhea, nausea, and vomiting. Raw shiitake can cause shiitake dermatitis, a distinctive skin eruption with whiplash-like marks, sometimes with whole-body symptoms.

Rarely, large unchewed pieces can block the intestines. Some people, especially those who handle shiitake mushrooms frequently, can develop allergic contact dermatitis, asthma, or other allergy symptoms from contact or inhalation.

Workers exposed to shiitake spores have occasionally developed hypersensitivity pneumonitis (a lung inflammation) with difficulty breathing, chest pain, dry cough, asthma, and rhinitis. Pregnancy and lactation safety data are not on file for this ingredient.
