B12 is generally very well tolerated when taken by mouth. Your body excretes excess B12 in the urine, so there's no established upper limit on how much you can safely take.

Most people don't experience side effects, though allergic reactions like rash or itching are possible — especially if you have a cobalt allergy, since cobalt is part of the B12 molecule. Very rarely, correcting severe B12 deficiency can cause a serious drop in potassium (low potassium), so if you're being treated for severe deficiency, your doctor should monitor your potassium levels.

B12 is needed in pregnancy and breastfeeding and is considered safe at normal recommended amounts — follow your provider's guidance on dosing.
