OptimALA contains 3 active ingredients: two stabilized forms of R-lipoic acid (NA-R-ALA and R-alpha lipoic acid cyclodextrin complex) and sesame extract. R-lipoic acid is the biologically active form of alpha-lipoic acid, a compound your body uses as a coenzyme in energy production and as an antioxidant.
Sesame extract adds additional compounds from sesame seed. The product also includes vegetable cellulose as an inactive ingredient to form the capsule.
Does it work?

Alpha-lipoic acid is rated possibly effective for diabetic neuropathy (nerve damage from diabetes) and possibly effective for high cholesterol (hyperlipidemia) and obesity. Sesame extract is rated possibly effective for high blood pressure, but possibly ineffective for cough.
For sesame's use in nonalcoholic fatty liver disease, HIV/AIDS-related wasting, Alzheimer disease, and anemia of chronic disease, the evidence isn't established well enough to rate it.
How safe is it?
Well-documented data
Alpha-lipoic acid is generally well tolerated in most adults, but it can lower blood sugar and works best under medical guidance. The most common side effects taken by mouth are headache, heartburn, nausea, and vomiting.
Rare case reports have raised concerns about a condition called insulin autoimmune syndrome. Skin rash and itching have also been reported.
Sesame is a major allergen—allergic reactions can range from skin swelling and rash to serious respiratory symptoms—and up to 0.5% of the U.S. population reports a sesame allergy. There isn't enough reliable safety data to recommend this product during pregnancy or while breastfeeding; talk with your doctor or pharmacist for personalized guidance.

How it works: OptimeALA is a novel and highly-bioavailable formulation of alpha lipoic acid (ALA). We take the extremely stable sodium salt of the active R-isomer of ALA, and combine it with a very bioavailable cyclodextrin complex of R-lipoic acid. This gives you two of the most potent and effective forms of R-ALA. In addition, it contains a sesame extract that is high in sesamin. When paired together, this provides beneficial improvements in lipid oxidation, blood glucose, antioxidant support, and energy metabolism.

The most serious concerns are Moderate-severity interactions with blood thinners and antiplatelet drugs (such as aspirin or warfarin); alpha-lipoic acid may increase bleeding risk by interfering with how your platelets clump together.

You'll also want to check before combining this with chemotherapy drugs—specifically alkylating agents and antitumor antibiotics. Alpha-lipoic acid's antioxidant effects are theoretically controversial during cancer treatment because they might reduce how well those drugs work.
Additionally, if you take thyroid hormone medication, alpha-lipoic acid may decrease its effectiveness. Sesame extract adds Moderate-level concerns with blood pressure medications and diabetes drugs; it can lower both blood pressure and blood sugar, which could be risky if you're already on medications for those conditions.
There's also a Moderate interaction with tamoxifen (a breast cancer drug) and a theoretical interaction with certain drugs metabolized by the liver enzyme CYP2C9.
Finally, alpha-lipoic acid carries a Minor interaction with diabetes medications—it may theoretically increase low blood sugar (hypoglycemia) risk, though larger studies haven't shown a strong effect. Altogether, these interactions span 656 individual medications.

Sesame extract
Antidiabetes Drugs
Taking sesame oil with antidiabetes drugs might increase the risk of hypoglycemia.
Clinical studies show that sesame oil can decrease plasma glucose and glycated hemoglobin (HbA1c) levels. Some clinical research in patients taking glibenclamide shows that using sesame oil or a blend of sesame oil and rice bran oil in place of other oil for cooking reduces plasma glucose more than glibenclamide alone. Monitor blood glucose levels closely. Dose adjustments might be necessary.
Antihypertensive Drugs
Taking sesame oil with antihypertensive drugs might increase the risk of hypotension.
Clinical research shows that replacing other cooking oil with sesame oil can lower systolic blood pressure (SBP) and diastolic blood pressure (DBP) in patients with or without hypertension. There is also some evidence that sesame oil has additive effects in patients also taking atenolol, nifedipine, and/or hydrochlorothiazide. In patients using nifedipine, using a blend of sesame oil and rice bran oil for cooking reduces both SBP and DBP more than nifedipine alone.
Cytochrome P450 2C9 (Cyp2C9) Substrates
Theoretically, sesame might increase the levels and clinical effects of CYP2C9 substrates.
In vitro, sesame inhibits CYP2C9. However, this interaction has not been reported in humans.
Tamoxifen (Nolvadex)
Theoretically, sesame might interfere with tamoxifen.
In animal research, sesame seed reduces the tumor-inhibitory effect of tamoxifen by reducing apoptosis and increasing proliferation. This interaction has not been reported in humans.
P-Glycoprotein Substrates
Theoretically, sesame might alter the transport of P-glycoprotein substrates.
In vitro research suggests that sesamin, a constituent of sesame, can inhibit the multi-drug transporter protein, P-glycoprotein. However, this interaction has not been reported in humans.
NA-R-ALA stabilized R-Lipoic Acid
Alkylating Agents
Theoretically, the antioxidant effects of alpha-lipoic acid might alter the effectiveness of alkylating agents.
The use of antioxidants like alpha-lipoic acid during chemotherapy is controversial. There are concerns that antioxidants could reduce the activity of chemotherapy drugs that generate free radicals. However, some researchers theorize that antioxidants might make chemotherapy more effective by reducing oxidative stress that might interfere with apoptosis (cell death) of cancer cells. More evidence is needed to determine what effect, if any, antioxidants such as alpha-lipoic acid have on chemotherapy. Advise patients to consult their oncologist before using alpha-lipoic acid.
Anticoagulant/Antiplatelet Drugs
Theoretically, alpha-lipoic acid may have antiplatelet effects and may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
In vitro, alpha-lipoic acid inhibits platelet aggregation.
Antitumor Antibiotics
Theoretically, the antioxidant effects of alpha-lipoic acid might alter the effectiveness of antitumor antibiotics.
The use of antioxidants like alpha-lipoic acid during chemotherapy is controversial. There are concerns that antioxidants could reduce the activity of antitumor antibiotic drugs, which work by generating free radicals. However, some researchers theorize that antioxidants might make chemotherapy more effective by reducing oxidative stress that might interfere with apoptosis (cell death) of cancer cells. More evidence is needed to determine what effect, if any, antioxidants such as alpha-lipoic acid have on chemotherapy involving antitumor antibiotics. Advise patients to consult their oncologist before using alpha-lipoic acid.
Thyroid Hormone
Theoretically, alpha-lipoic acid might decrease the effects of thyroid hormone drugs.
Animal research suggests that co-administration of thyroxine with alpha-lipoic acid reduces conversion into the active T3 form.
Antidiabetes Drugs
Theoretically, taking alpha-lipoic acid with antidiabetes drugs might increase the risk of hypoglycemia.
Although some small clinical studies have suggested that alpha-lipoic acid can lower blood glucose levels, larger clinical studies in patients with diabetes have shown no clinically meaningful effect. Additionally, co-administration of single doses of alpha-lipoic acid and glyburide or acarbose did not cause detectable drug interactions in healthy volunteers.
