Lactase is generally well tolerated when used as directed. Rarely, people can develop an allergic reaction to lactase itself — allergic rhinoconjunctivitis (nasal and eye symptoms) and contact dermatitis have been reported in occupational settings with powdered lactase exposure.

Lactobacillus acidophilus is generally well tolerated in healthy adults and children. Gastrointestinal side effects like stomach discomfort, bloating, gas, diarrhea, and burping can occur, but they're uncommon.

There's a theoretical concern that L. acidophilus might cause infection in some patients — cases of bacteremia and other serious infections have been documented, though most are thought to involve bacteria from other body locations, not from the probiotic itself. For pregnancy, lactase is rated possibly safe.
