Saw palmetto is generally well tolerated, and any side effects tend to be mild and reversible. The most common ones reported in trials are abdominal pain, constipation, decreased libido, diarrhea, dizziness, fatigue, headache, nausea, sinus irritation, and vomiting.

Rarely, a fixed drug eruption (localized blisters and erosions on the skin) has been reported, and very occasionally, heart-related symptoms like irregular heartbeat or high blood pressure have occurred — though these are uncommon.
