Suprema Dophilus is a 10-ingredient probiotic and prebiotic formula. It contains a proprietary probiotic blend featuring live bacterial strains including Bifidobacterium longum, Lactobacillus acidophilus, and Bifidobacterium bifidum, plus five other Lactobacillus and Bifidobacterium strains.
These are meant to support your gut bacteria. The formula also includes a prebiotic blend with fructooligosaccharides (FOS) and larch arabinogalactan (FiberAid), which act as food for the beneficial bacteria.
The remaining ingredients are inert capsule and flow agents: cellulose, modified cellulose, water, silica, magnesium stearate, and calcium silicate.

The evidence for this product's ingredients is mixed. Lactobacillus acidophilus is possibly effective for irritable bowel syndrome, H. pylori infection, antibiotic-associated diarrhea, and bacterial vaginosis.
Bifidobacterium bifidum is possibly effective for irritable bowel syndrome and respiratory tract infections. For the other probiotic strains and the prebiotic ingredients—FOS and larch arabinogalactan—the data we hold shows insufficient reliable evidence to rate their effectiveness for the conditions listed.
That doesn't mean they don't work; it means the research isn't established enough in our records to call it either way.
How safe is it?
Well-documented data
Suprema Dophilus is generally well tolerated in healthy people. The most common side effects are gastrointestinal: abdominal discomfort, bloating, flatulence, and gas—especially with the FOS and larch arabinogalactan components.
These are usually mild, particularly at lower doses. Rare cases of dry cough and gastrointestinal issues like constipation have been reported with the Bifidobacterium strains.
Very rarely, live probiotic bacteria have caused serious infections (bacteremia or sepsis) in critically ill, premature, or severely immune-compromised patients, so caution is warranted if you're in one of those groups. The safety data for supplement-level doses of these ingredients is limited, especially in breastfeeding; Bifidobacterium longum is rated possibly safe in pregnancy and lactation, but safety information isn't on file for the other strains or for FOS and larch arabinogalactan during breastfeeding.

The most serious concern is with antibiotic drugs (Moderate severity): several of the live bacterial ingredients—Bifidobacterium longum, Lactobacillus acidophilus, and Bifidobacterium bifidum—may lose effectiveness if taken at the same time as antibiotics, since the antibiotics can kill the beneficial organisms before they establish themselves. Antibiotic Drugs
Theoretically, taking Bifidobacterium longum with antibiotic drugs might decrease the effectiveness of B. longum.
Since B. longum preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. longum preparations by at least 2 hours.
Lactobacillus acidophilus LA-14
Antibiotic Drugs
Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
Bifidobacterium bifidum BB-2
Antibiotic Drugs
Theoretically, taking Bifidobacterium. bifidum with antibiotic drugs might decrease the effectiveness of B. bifidum.
Since B. bifidum preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. bifidum preparations by at least 2 hours.

Immunosuppressant drugs include azathioprine (Imuran), basiliximab (Simulect), cyclosporine (Neoral, Sandimmune), daclizumab (Zenapax), muromonab-CD3 (OKT3, Orthoclone OKT3), mycophenolate (CellCept), tacrolimus (FK506, Prograf), sirolimus (Rapamune), prednisone (Deltasone, Orasone), corticosteroids (glucocorticoids), and other drugs.
