Synerpan-5 contains four active ingredients, all digestive enzymes. Bromelain is an enzyme from pineapple that breaks down proteins.
Protease and lipase are also protein- and fat-digesting enzymes, respectively, often derived from pancreatic tissue. Amylase is an enzyme that breaks down carbohydrates.
Together, they're meant to support your digestive system. The capsule itself is made from gelatin and cellulose, with vegetable stearate added as a flow agent.

The evidence for these enzymes in treating specific conditions is not established in our data. Bromelain has been studied for hay fever, burns, cancer, chemotherapy side effects, circulation problems, and diabetes, but the research is insufficient to rate how well it works for any of these.
Lipase similarly lacks reliable evidence for inflammatory bowel disease or indigestion. Our facts don't include effectiveness ratings for protease or amylase.

How safe is it?
Well-documented data
Bromelain is generally well tolerated when taken by mouth short-term, though diarrhea, gas, stomach upset, and headache can occur. If inhaled as a powder, bromelain can trigger allergic reactions including asthma, throat itching, and eye irritation—occupational exposure is the main concern here.
Protease and lipase are also generally well tolerated, though digestive upset and allergic reactions are possible; protease exposure in occupational settings has caused asthma and allergic rhinitis. We have no data on bromelain's safety in pregnancy—the safety data advises against it.
Breastfeeding safety is unknown for all three enzymes we have pregnancy/lactation data for, so medical advice is recommended before use. Lipase requires special caution in serious digestive conditions and should only be used under medical guidance.

The most serious concern is with blood thinners and antiplatelet drugs (like warfarin, aspirin, and clopidogrel). Bromelain may have mild blood-thinning effects itself and could increase your risk of bleeding when combined with these drugs.
One case report noted minor bruising in a patient taking bromelain with naproxen (an anti-inflammatory pain reliever).

Bromelain may also theoretically increase the absorption of tetracycline antibiotics (a class of oral antibiotics). However, a study in healthy adults found no meaningful change in tetracycline blood levels when taken with bromelain, so this is a minor concern.
Two other active ingredients in this product—protease and lipase—have been checked and show no documented interactions with medications. We could not check amylase because we hold no data for it.

Bromelain
Anticoagulant/Antiplatelet Drugs
Bromelain may have antiplatelet effects and may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
There is one case report of a patient experiencing minor bruising while taking bromelain with naproxen. Bromelain is thought to have antiplatelet activity. Whether this interaction is of concern with topical bromelain is unclear. Interference with coagulation of burn wounds has been reported in a patient receiving bromelain-based enzymatic debridement. However, observational research has found that topical bromelain debridement is not associated with increases or decreases in laboratory markers of coagulation when compared with surgical debridement.
Tetracycline Antibiotics
Theoretically, bromelain might increase levels of tetracycline antibiotics.
Laboratory research suggests that bromelain might increase the absorption of tetracycline antibiotics. However, a study in healthy adults reported no difference in tetracycline plasma levels when a 500 mg dose was taken with or without bromelain 80 mg.
