Ther-Biotic Synbiotic contains 8 active ingredients: a blend of six Lactobacillus and Bifidobacterium probiotic strains plus Sunfiber, a soluble fiber. The Lactobacillus acidophilus DDS-1 and Bifidobacterium bifidum are the strains we have data for; the others are Lactobacillus rhamnosus GG, Lactobacillus plantarum UALp-05, Bifidobacterium animalis lactis UABla-12, Lactobacillus reuteri UALre-16, and Lactobacillus paracasei UALpc-04.
The inactive ingredients include the capsule material (hydroxypropyl methylcellulose), thickeners, and preservatives.
Does it work?
Moderate evidence
Lactobacillus acidophilus is possibly effective for irritable bowel syndrome (IBS), antibiotic-associated diarrhea, Helicobacter pylori infection, and bacterial vaginosis. Bifidobacterium bifidum is possibly effective for IBS and respiratory tract infections.
For other conditions listed — hay fever, Alzheimer disease, and generalized anxiety — the evidence isn't established enough to rate. We don't have effectiveness data on the other probiotic strains or Sunfiber in this product.
How safe is it?
Well-documented data
Lactobacillus acidophilus and Bifidobacterium bifidum are generally well tolerated in healthy adults and children. Gastrointestinal side effects like bloating, flatulence, abdominal discomfort, and diarrhea can occur with probiotics, though these are uncommon.
A very small number of trial participants reported rash or itching, though it's unclear if the probiotics were responsible. Because these are live organisms, there is a theoretical risk of infection in people with severely weakened immune systems; those with compromised immunity should check with their doctor before starting.
Pregnancy and lactation safety data aren't on file for the ingredients we could check.

Antibiotic Drugs
Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
