Moderate interactions span multiple drug types. American ginseng may also interfere with immunosuppressants, increase low blood sugar (hypoglycemia) risk with diabetes drugs, and theoretically interfere with MAOI antidepressants.
Reishi mushroom may add to the blood-pressure-lowering effect of blood pressure drugs, increase bleeding risk at higher doses when combined with blood thinners (anticoagulants) or antiplatelet drugs, and theoretically add blood-sugar-lowering effects with diabetes drugs. Maitake mushroom carries similar moderate concerns: it may lower blood pressure further with blood pressure drugs, raise hypoglycemia risk with diabetes medications, and possibly strengthen warfarin's effect.
Shiitake mushroom may interfere with immunosuppressants and theoretically affect drugs broken down by the liver enzyme CYP2D6. Beta-glucan also may lower blood pressure further with blood pressure drugs and theoretically interfere with immunosuppressants.

Reishi mushroom 10:1 extract
Anticoagulant/Antiplatelet Drugs
Theoretically, high doses of reishi mushroom might increase the risk of bleeding.
A dose of 1.5 grams daily of reishi mushroom does not seem to decrease platelet aggregation, but a higher dose of 3 grams daily does.
Antidiabetes Drugs
Theoretically, reishi mushroom might have additive effects with antidiabetes drugs.
Animal research suggests that reishi mushroom decreases blood sugar. However, in patients with type 2 diabetes, taking reishi mushroom does not reduce fasting glucose levels, and its effects on glycated hemoglobin are inconsistent.
Antihypertensive Drugs
Theoretically, concurrent use of reishi mushroom with antihypertensive drugs might increase the risk of hypotension.
Reishi mushroom has shown hypotensive activity in animal research. Clinical evidence suggests that reishi mushroom reduces blood pressure in some, but not all, patients with hypertension.
Shitake mushroom 10:1 extract
Cytochrome P450 2D6 (Cyp2D6) Substrates
Theoretically, shiitake mushroom might decrease levels of drugs metabolized by CYP2D6.
In vitro studies suggest that the shiitake mushroom extract AHCC might induce the CYP2D6 enzyme. This effect has not been reported in humans.
Immunosuppressants
Theoretically, taking shiitake mushroom might decrease the effects of immunosuppressive therapy.
In vitro evidence suggests that shiitake mushroom extracts stimulate immune function.
Beta 1,3 Glucan
Antihypertensive Drugs
Theoretically, taking beta-glucans with antihypertensive drugs might increase the risk of hypotension.
Clinical research shows that taking beta-glucans may reduce systolic and diastolic blood pressure in some hypertensive individuals.
Immunosuppressants
Theoretically, beta-glucans might interfere with immunosuppressive therapy.
Some clinical research shows that beta-glucans have immunostimulant effects.
Maitake mushroom 8:1 extract
Antidiabetes Drugs
Theoretically, combining maitake mushroom with antidiabetes drugs might increase the risk of hypoglycemia.
Clinical research shows that taking maitake mushroom polysaccharide (MMP) can lower blood glucose levels in patients with types 2 diabetes.
Antihypertensive Drugs
Theoretically, combining maitake mushroom with antihypertensive drugs might increase the risk of hypotension.
Animal research shows that maitake mushroom can lower blood pressure.
Warfarin (Coumadin)
There is limited evidence that maitake mushroom may increase the anticoagulant effects of warfarin.
In a case report, a patient previously stabilized on warfarin developed an elevated international normalized ratio (INR) of 5.1 after taking maitake mushroom (Grifron-Pro Maitake D-Fraction) 1 drop/kg daily in three divided doses for one week. The elevated INR resolved after holding warfarin for two days, then reducing the dose by 11%. It is thought that the beta-glucan constituent of maitake mushroom might cause warfarin dissociation from proteins, resulting in increased free warfarin levels and increased warfarin effects.
American Ginseng root 4:1 extract
Warfarin (Coumadin)
American ginseng seems to decrease the effectiveness of warfarin therapy.
Healthy patients receiving warfarin 5 mg daily, who also take American ginseng 1 gram twice daily, seem to have a significantly reduced international normalized ratio (INR).
Antidiabetes Drugs
Theoretically, taking American ginseng with antidiabetes drugs might increase the risk of hypoglycemia.
American ginseng seems to lower postprandial blood glucose. Theoretically, concomitant use with antidiabetes drugs might enhance blood glucose lowering effects and possibly cause hypoglycemia.
Immunosuppressants
Theoretically, American ginseng use might interfere with immunosuppressive therapy.
American ginseng seems to stimulate immune function. Theoretically, American ginseng might decrease the effectiveness of immunosuppressant drugs.
Monoamine Oxidase Inhibitors (Maois)
Theoretically, American ginseng can interfere with MAOI therapy.
There is one case report of insomnia, headache, and tremors when an unspecified ginseng product was used with phenelzine (Nardil), an MAOI. There is also one case report of hypomania when an unspecified ginseng product was used with phenelzine. Theoretically, American ginseng may interfere with MAOI therapy.
