Here's the thinking behind this pairing. Azathioprine works by quieting down your immune system, which is exactly what you want when you're protecting a transplanted kidney or calming overactive joints in rheumatoid arthritis. The concern is that some beta-glucans, mainly the ones from yeast and mushrooms, may nudge the immune system in the opposite direction and rev it up a bit. In theory, that could work against what azathioprine is trying to do.

I want to be honest about how soft this evidence is. It mostly comes from lab and animal studies, so it's more of a theoretical caution than a proven problem. The oat and barley beta-glucans people take for cholesterol are far less likely to matter here. Still, since azathioprine is serious medicine, run any beta-glucan supplement past your transplant team or pharmacist first.

The strongest evidence supports the oat and barley types for modestly lowering cholesterol and blood sugar, while immune-related claims from yeast and mushroom sources are far less proven. They are generally well tolerated, but check with your pharmacist before starting any supplement.

About Azathioprine

What Is Azathioprine Used For?

Brand names: Imuran, Azamune

Azathioprine is used alongside other medications to help prevent the body from rejecting a kidney transplant (renal homotransplantation). It is also used to treat active rheumatoid arthritis — reducing signs and symptoms in adults who have not responded well enough to other treatments.

Other pain relievers such as aspirin, NSAIDs, or low-dose steroids may be continued alongside azathioprine for rheumatoid arthritis. Combining azathioprine with disease-modifying antirheumatic drugs (DMARDs) has not been well studied and is not recommended.
