If you're taking blood pressure medication (antihypertensive drugs), adding tribulus could lower your blood pressure further, raising the risk of hypotension (abnormally low blood pressure). The herb works partly by blocking an enzyme that normally helps regulate blood pressure, and it's shown blood-pressure-lowering effects in people with elevated readings.
Similarly, if you take diabetes medication, tribulus may lower your blood sugar more than your medication alone, increasing the risk of hypoglycemia (low blood sugar). Research shows it can reduce blood glucose in adults with type 2 diabetes who are already on diabetes drugs.
Tribulus may also affect how your body handles lithium. Because it appears to have diuretic (water-shedding) properties, it might reduce how much lithium your kidneys clear, causing levels to rise and potentially requiring a dose adjustment.

Antidiabetes Drugs
Taking tribulus with antidiabetes drugs might increase the risk of hypoglycemia.
Clinical research shows that Tribulus can lower blood glucose levels in adults with type 2 diabetes who are taking antidiabetes medications.
Antihypertensive Drugs
Theoretically, taking tribulus with antihypertensive drugs might increase the risk of hypotension.
Animal research shows that tribulus can lower blood pressure by inhibiting angiotensin-converting enzyme (ACE). Tribulus has also demonstrated hypotensive effects in pre-hypertensive adults.
Lithium
Theoretically, tribulus might increase the levels and clinical effects of lithium.
