Twelve Rivers Mycomedic is a powder blend of eight mushroom fruiting body extracts. The active ingredients are Chaga, Cordyceps, Fu Ling (poria mushroom), Maitake, Reishi, Royal Sun Agaricus, Turkey Tail, and Willow Bracket.
Each is included as a concentrated extract rather than whole mushroom material. The product contains no other listed inactive ingredients.

Not established
The evidence base for most of these mushrooms is limited. Cordyceps is rated possibly ineffective for athletic performance.
Reishi is rated possibly ineffective for high cholesterol (hyperlipidemia). For the remaining conditions studied—including cardiovascular disease, diabetes, COVID-19, sexual dysfunction, asthma, chronic fatigue, and various forms of cancer—the data we hold shows insufficient reliable evidence to rate whether these mushrooms work.

How safe is it?
Well-documented data
Chaga, Cordyceps, Fu Ling, Maitake, and Reishi are generally well tolerated short-term, though long-term safety data are limited for all of them. Chaga carries a specific concern: excessive consumption can cause kidney damage due to its high oxalate content; two cases of kidney failure and one of acute kidney injury have been reported.
Cordyceps, Maitake, and Reishi may cause gastrointestinal upset—diarrhea, constipation, abdominal discomfort, nausea, and stomach upset—though these were uncommon in trials and sometimes resolved when the mushroom was taken with food. Reishi may also cause dizziness, dry mouth, itching, rash, sore throat, or runny nose.
One case of liver inflammation (cholestatic hepatitis) was reported with a cordyceps product, though whether cordyceps itself was the cause is unclear. Allergic reactions, including allergic rhinitis and allergic asthma, have been reported rarely with poria mushroom, and one case of rash and itching occurred with Maitake.
The safety data for Royal Sun Agaricus, Turkey Tail, and Willow Bracket are not on file.

The most serious concern is a moderate-severity interaction with blood thinners (anticoagulants) and antiplatelet drugs like aspirin and warfarin. Chaga, Cordyceps, and Reishi all may theoretically increase bleeding risk through different mechanisms—chaga and cordyceps by inhibiting platelet function, and reishi at higher doses by the same route.
One case report documented a patient on warfarin who developed an elevated INR after taking Maitake mushroom.

Several other drug types are affected at moderate severity. Chaga, Cordyceps, and Maitake may interfere with immunosuppressants like cyclosporine.
Chaga, Maitake, and Reishi may increase the risk of low blood sugar (hypoglycemia) if combined with diabetes medications. Maitake and Reishi may also lower blood pressure in combination with blood pressure–lowering drugs.
Fu Ling (poria mushroom) interacts with sedatives (CNS depressants), anticholinergic drugs, and cholinergic drugs—all at moderate severity—through effects on the nervous system.
Cordyceps carries a minor interaction with testosterone at the hormonal level. We could not check Royal Sun Agaricus, Turkey Tail, or Willow Bracket fruiting body extracts—interaction data is not on file for them.

Fu Ling Fruiting Body Extract
Anticholinergic Drugs
Theoretically, poria mushroom might decrease the clinical effects of anticholinergic drugs.
In animal research, poria mushroom essential oil reduces acetylcholinesterase activity. This interaction has not been shown in humans.
Cholinergic Drugs
Theoretically, poria mushroom might have additive effects when used with cholinergic drugs.
In animal research, poria mushroom essential oil reduces acetylcholinesterase activity. This interaction has not been shown in humans.
Cns Depressants
Theoretically, taking poria mushroom extract may enhance the therapeutic and adverse effects of sedatives.
Animal research shows that poria mushroom extract has sedative properties. This interaction has not been shown in humans.
Reishi Fruiting Body Extract
Anticoagulant/Antiplatelet Drugs
Theoretically, high doses of reishi mushroom might increase the risk of bleeding.
A dose of 1.5 grams daily of reishi mushroom does not seem to decrease platelet aggregation, but a higher dose of 3 grams daily does.
Antidiabetes Drugs
Theoretically, reishi mushroom might have additive effects with antidiabetes drugs.
Animal research suggests that reishi mushroom decreases blood sugar. However, in patients with type 2 diabetes, taking reishi mushroom does not reduce fasting glucose levels, and its effects on glycated hemoglobin are inconsistent.
Antihypertensive Drugs
Theoretically, concurrent use of reishi mushroom with antihypertensive drugs might increase the risk of hypotension.
Reishi mushroom has shown hypotensive activity in animal research. Clinical evidence suggests that reishi mushroom reduces blood pressure in some, but not all, patients with hypertension.
Chaga Fruiting Body Extract
Anticoagulant/Antiplatelet Drugs
Theoretically, chaga may increase the risk of bleeding if used with anticoagulant or antiplatelet drugs.
In vitro and animal research suggests that chaga extract can inhibit platelet aggregation. This effect has not been reported in humans.
Antidiabetes Drugs
Theoretically, taking chaga with antidiabetes drugs might increase the risk of hypoglycemia.
Animal research suggests that chaga might decrease blood glucose levels and increase insulin levels. This has not been reported in humans.
Immunosuppressants
Theoretically, chaga might interfere with immunosuppressive therapy.
In vitro research suggests that certain constituents of chaga stimulate immune function. This has not been reported in humans.
Maitake Fruiting Body Extract
Antidiabetes Drugs
Theoretically, combining maitake mushroom with antidiabetes drugs might increase the risk of hypoglycemia.
Clinical research shows that taking maitake mushroom polysaccharide (MMP) can lower blood glucose levels in patients with types 2 diabetes.
Antihypertensive Drugs
Theoretically, combining maitake mushroom with antihypertensive drugs might increase the risk of hypotension.
Animal research shows that maitake mushroom can lower blood pressure.
Warfarin (Coumadin)
There is limited evidence that maitake mushroom may increase the anticoagulant effects of warfarin.
In a case report, a patient previously stabilized on warfarin developed an elevated international normalized ratio (INR) of 5.1 after taking maitake mushroom (Grifron-Pro Maitake D-Fraction) 1 drop/kg daily in three divided doses for one week. The elevated INR resolved after holding warfarin for two days, then reducing the dose by 11%. It is thought that the beta-glucan constituent of maitake mushroom might cause warfarin dissociation from proteins, resulting in increased free warfarin levels and increased warfarin effects.
Cordyceps Fruiting Body Extract
Anticoagulant/Antiplatelet Drugs
Theoretically, cordyceps may increase the risk of bleeding when used with antiplatelet or anticoagulant drugs.
In vitro and animal research suggests that cordyceps extract inhibits platelet aggregation and function. However, this interaction has not been reported in humans.
Immunosuppressants
Theoretically, concurrent use of cordyceps might interfere with immunosuppressive therapy.
Animal and in vitro research suggests that cordyceps stimulates the immune system. However, limited clinical research suggests that taking cordyceps may lower the necessary therapeutic dose of the immunosuppressant cyclosporine, which suggests that cordyceps may have an immunosuppressive effect.
Testosterone
Theoretically, concurrent use of cordyceps and testosterone might have additive effects.
Animal research suggests that cordyceps can increase testosterone levels. The clinical significance of this finding is unclear.
