Lactobacillus acidophilus is generally well tolerated in healthy adults and children. Because these are live organisms, there's a theoretical concern they could cause infection in some patients — cases of bacteremia (bacteria in the bloodstream), sepsis, and other serious infections have been rarely documented, though most are thought to happen when bacteria move from other parts of your body where they naturally live.

When taken with other probiotics, gastrointestinal side effects like stomach discomfort, abdominal pain, gas, bloating, diarrhea, vomiting, and burping can occur, but they're uncommon. In one trial, a probiotic combination including L. acidophilus was linked to two cases of rash and one case of itching, though it's unclear whether L. acidophilus itself was responsible.
