Lactobacillus acidophilus is generally well tolerated in healthy adults and children. Since this product contains live, active microorganisms, there's a theoretical concern that in rare cases they might cause infection in people with certain health conditions — though most documented cases appear linked to bacteria from other parts of the body, not the probiotic itself.

When taking Lactobacillus acidophilus with other probiotics, a small number of people reported mild gastrointestinal side effects like stomach discomfort, bloating, gas, or diarrhea, but these are uncommon. In one trial, a rash and itching were reported, though it wasn't clear whether they came from the L. acidophilus or another factor.
