Herb Pharm's Umckaloabo is a single-ingredient liquid extract of Pelargonium sidoides root (also called umckaloabo) that has documented interactions with two medication types.
The most serious concern is with immunosuppressants — the kind you'd take after an organ transplant or for severe autoimmune conditions. In vitro research suggests this extract might stimulate your immune system and theoretically reduce how well those drugs work, though clinical data in humans is limited.
Additionally, there's a minor interaction with blood thinners (anticoagulants) and antiplatelet drugs like aspirin or warfarin. The extract contains coumarin, which theoretically could increase bleeding risk, but animal studies suggest it doesn't actually interact with warfarin or impair blood clotting the way you'd expect — so the real-world risk appears lower than the theory suggests.

Umckaloabo Root Extract
Immunosuppressants
Theoretically, Pelargonium sidoides might decrease the effectiveness of immunosuppressant therapy.
In vitro research suggests that Pelargonium sidoides has immunostimulant effects.
Anticoagulant/Antiplatelet Drugs
Theoretically, Pelargonium sidoides might increase the risk of bleeding when taken with anticoagulant and antiplatelet drugs.
Pelargonium sidoides contains coumarin, which may reduce platelet aggregation. However, in animal models, Pelargonium sidoides extract does not appear to inhibit coagulation or interact with warfarin. This interaction has not been documented in humans.
