Theoretically, Pelargonium sidoides might decrease the effectiveness of immunosuppressant therapy.

Here's the thinking behind this one. Azathioprine works by turning down your immune system, which is exactly what you want when you're protecting a transplanted kidney or calming down severe rheumatoid arthritis. Umckaloabo, on the other hand, has lab and animal data suggesting it may stimulate the immune system. So in theory, the two pull in opposite directions, and the worry is that the herb could blunt how well your azathioprine holds your immune system in check.

I want to be honest that this is theoretical, based on how the supplement behaves in a test tube, not on real patient cases. Still, if you've had a transplant, even a small dip in protection isn't worth gambling on. Please loop in your pharmacist or doctor before adding Umckaloabo so they can weigh it for your situation.

Umckaloabo is a root extract from a South African geranium that is most studied for short-term relief of acute bronchitis and other respiratory infections. Some clinical research suggests it may help symptoms resolve a bit faster, but the quality of evidence is mixed and it is not a cure. Talk to your pharmacist or doctor before using it, especially if you are pregnant, breastfeeding, or have a chronic illness.

About Azathioprine

What Is Azathioprine Used For?

Brand names: Imuran, Azamune

Azathioprine is used alongside other medications to help prevent the body from rejecting a kidney transplant (renal homotransplantation). It is also used to treat active rheumatoid arthritis — reducing signs and symptoms in adults who have not responded well enough to other treatments.

Other pain relievers such as aspirin, NSAIDs, or low-dose steroids may be continued alongside azathioprine for rheumatoid arthritis. Combining azathioprine with disease-modifying antirheumatic drugs (DMARDs) has not been well studied and is not recommended.
