Theoretically, Pelargonium sidoides might decrease the effectiveness of immunosuppressant therapy.

Here's the idea behind this pairing. Abrocitinib (Cibinqo) works by dialing down your immune system to calm the inflammation that drives eczema. Umckaloabo, on the other hand, seems to gently rev the immune system up in lab studies. So in theory, the supplement could push in the opposite direction from your medication and make it work a little less well, meaning your skin itching, dryness, and rashes might not stay as controlled.

I want to be honest though: this concern comes only from test-tube research, not from people actually taking both together, so it's a theoretical worry rather than a proven problem. Abrocitinib isn't a prodrug, so this is about competing effects, not activation. Before you add Umckaloabo, please run it by your pharmacist or dermatologist so they can keep an eye on how your eczema is responding.

Theoretically, Pelargonium sidoides might increase the risk of bleeding when taken with anticoagulant and antiplatelet drugs.
Pelargonium sidoides contains coumarin, which may reduce platelet aggregation. However, in animal models, Pelargonium sidoides extract does not appear to inhibit coagulation or interact with warfarin. This interaction has not been documented in humans.

Here's the thing to keep in mind with this pairing. Umckaloabo (the geranium root extract) contains coumarins that might make platelets a little less sticky, which could in theory nudge up your bleeding risk. Abrocitinib (Cibinqo), your JAK inhibitor for eczema, can on its own occasionally affect blood counts, so the worry is that the two together might make minor bleeding or bruising a touch more likely.

That said, this is a theoretical, low-level concern. It's based on lab and animal work, animal studies didn't even show a real clotting effect, and it has never been reported in people. So I wouldn't lose sleep over it. Still, if you notice easy bruising, nosebleeds, or bleeding gums, or you take other blood thinners, give your pharmacist or doctor a quick heads-up first.

What Is Abrocitinib Used For?

Brand name: Cibinqo

Cibinqo (abrocitinib) is approved to treat adults and children aged 12 and older who have moderate-to-severe atopic dermatitis (eczema) that has not been adequately controlled with other systemic treatments, including biologics — or when those treatments are not appropriate.

Cibinqo is specifically for refractory cases, meaning it is intended for people whose eczema has not responded well enough to other options. It is not recommended to be used alongside other JAK inhibitors, biologic immunomodulators, or other immunosuppressants.
