Here is the interaction we found between these two, with the evidence behind it.
IMMUNOSUPPRESSANTS
Why this applies to Elranatamab
Elranatamab falls under IMMUNOSUPPRESSANTS — the group Umckaloabo interacts with.
In vitro research suggests that Pelargonium sidoides has immunostimulant effects.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thinking behind pairing these two. Elranatamab (Elrexfio) is a powerful immune therapy used for multiple myeloma. It works by revving up your T cells to attack cancer, but it also dampens parts of your immune system, which is why doctors watch closely for infections. Umckaloabo, on the other hand, contains plant compounds that lab studies suggest may stimulate the immune system.
So in theory, an immune-boosting herb could work against the direction Elranatamab is trying to go, or add unpredictable immune activity. I'll be honest with you: this concern comes only from test-tube research, not from real patients, so it's more of a caution than a proven problem. Still, with a serious drug like this, I'd hold off and check with your oncologist or pharmacist first.

From our pharmacist’s overview on the Elranatamab page
“Elranatamab (Elrexfio) fights multiple myeloma by recruiting the immune system against the cancer, and that mechanism is exactly why I want people on it to read this page carefully rather than skim it. Most of the interactions here involve herbs and mushroom extracts that stimulate immune activity, which could in theory interfere with…”

- Upper respiratory infections
Umckaloabo is a root extract from a South African geranium that is most studied for short-term relief of acute bronchitis and other respiratory infections.
