Ibuprofen falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Umckaloabo interacts with.

Theoretically, Pelargonium sidoides might increase the risk of bleeding when taken with anticoagulant and antiplatelet drugs.
Pelargonium sidoides contains coumarin, which may reduce platelet aggregation. However, in animal models, Pelargonium sidoides extract does not appear to inhibit coagulation or interact with warfarin. This interaction has not been documented in humans.

Here's the gist: ibuprofen already has a mild blood-thinning side, since it slows the platelets that help your blood clot. Umckaloabo contains a little coumarin, and on paper that might also nudge platelets in the same direction. Stack the two and the theory is you could bruise or bleed a touch more easily, say a nosebleed that lingers or a cut that's slow to stop.

I want to be honest about how thin this concern is. It's theoretical only, based on lab chemistry, not real-world cases. In animal studies the extract didn't actually interfere with clotting, and it's never been reported in people. For most folks taking ibuprofen now and then for a cold or aches, this isn't something to lose sleep over. If you're on a true blood thinner too, or have a bleeding condition, run it by your pharmacist first.

What Is Ibuprofen Used For?

Brand names: Motrin, Children's Motrin, Advil, Children's Advil, Fenbid Forte Gel, Ibugel Forte, Arthrofen, Ebufac, Rimafen, Calprofen, Fenpaed, Galprofen, Nurofen for Children, Orbifen for Children, Brufen, Brufen Retard, Fenbid, Tab-Profen, Ibuprofen, Calodor

Oral ibuprofen products (tablets, coated tablets, and liquid-filled capsules) are used to temporarily relieve minor aches and pains from headache, backache, toothache, muscular aches, menstrual cramps, minor arthritis pain, and the common cold, and to temporarily reduce fever. The oral prescription tablet (Ibu) is also indicated for the signs and symptoms of rheumatoid arthritis and osteoarthritis, mild to moderate pain, and primary dysmenorrhea (painful periods).

Caldolor, an intravenous (IV) form, is approved for adults and children 3 months and older to manage mild to moderate pain, to manage moderate to severe pain alongside opioid pain medicines, and to reduce fever. A separate intravenous solution, Ibuprofen Lysine, is used specifically in premature infants to close a patent ductus arteriosus (PDA) — an abnormal blood vessel connection near the heart — when standard medical management has not worked.
