HelloPharmacist Scorecard of Uva-Ursi by Herb Pharm
Our pharmacy team’s full take, with four database checks built into the cards below — a summary of what is known, not a grade of the product itself.
What’s inside
Full disclosure
This product contains one active ingredient: uva-ursi, a plant whose leaves have traditionally been used for urinary tract support. The liquid form also includes certified organic cane alcohol, distilled water, and certified organic vegetable glycerin as inactive ingredients that help deliver and preserve the herb.
Does it work?
Leans against
The evidence for uva-ursi is mixed. For urinary tract infections (UTIs), the data suggests it's possibly ineffective — meaning the science doesn't support it for that use.
For bronchitis and benign prostatic hyperplasia (enlarged prostate), there isn't enough reliable evidence to rate how well it works. We hold no effectiveness data for any other conditions.
How safe is it?
Well-documented data
Uva-ursi is generally well tolerated at low doses over the short term. The most common side effects are mild — nausea, vomiting, diarrhea, and stomach upset.
But here's the caution: don't use it long-term. At very high doses (20 grams of dried herb or more), it can cause serious effects like convulsions, delirium, shortness of breath, and dangerously low oxygen levels.
Doses of 30 grams or more may be fatal. Pregnancy and breastfeeding: the safety data advises against use during pregnancy because uva-ursi may stimulate the uterus and we don't have enough safety information.
There also isn't enough data to know it's safe while breastfeeding, so avoid it then too.
Meds to double-check
Moderate interaction found
Before you start, check whether you take lithium; medications metabolized through CYP2C19 or CYP3A4 (ask your pharmacist which yours are); glucuronidated drugs; urinary acidifying agents; or P-glycoprotein substrates. If you do, run them through the checker below — uva-ursi may alter how your body handles them.
The bottom line
Scorecard at a glanceFully disclosed formula with graded evidence leaning against its stated purpose. Moderate medication interactions have been identified, and safety information is well characterized.
Uva-ursi is traditionally used for urinary tract issues, but the evidence doesn't support that use well. If you take lithium, medications processed by your liver's major enzyme systems, or certain other drugs, check with your pharmacist first — several medication types can interact.
Keep it short-term and at normal doses, and skip it if you're pregnant or nursing.

Uva-Ursi by Herb Pharm Drug Interactions
HelloPharmacist Interaction Report
Uva-Ursi by Herb Pharm contains a single active ingredient — uva-ursi — that interacts with several medication types.
The most serious concern is with lithium (used for mood disorders): uva-ursi may have mild diuretic properties that could reduce your kidneys' ability to clear lithium, potentially raising it to unsafe levels and requiring a dose adjustment.
Read the full breakdown — every affected drug type, severity by severity
Uva-ursi also theoretically slows how your liver metabolizes drugs processed through two major enzyme systems: CYP2C19 and CYP3A4 substrates (Moderate severity for both). It may also increase levels of glucuronidated drugs — those your body clears through a different metabolic pathway (Moderate).
These effects haven't been seen in humans, only in lab studies, but they're worth checking. Additionally, uva-ursi may interfere with P-glycoprotein substrates (Minor), a protein that transports certain drugs in and out of cells.
One more thing: if you take medications that acidify your urine (like some antiinfectives), they may reduce uva-ursi's effectiveness in the urinary tract. Altogether, these interactions span 790 individual medications.

Uva-Ursi
Cytochrome P450 2C19 (Cyp2C19) Substrates
Theoretically, uva ursi may decrease the metabolism of CYP2C19 substrates.
In vitro, uva ursi appears to inhibit cytochrome CYP2C19. This effect has not been reported in humans.
Cytochrome P450 3A4 (Cyp3A4) Substrates
Theoretically, uva ursi may decrease the metabolism of CYP3A4 substrates.
In vitro, uva ursi appears to inhibit CYP3A4. This effect has not been reported in humans.
Glucuronidated Drugs
Theoretically, uva ursi may increase levels of drugs metabolized by glucuronidation.
In vitro, uva ursi extract appears to strongly inhibit UDP-glucuronosyltransferase (UGT) 1A1 (UGT1A1). However, uva ursi extract does not appear to inhibit UGT1A1 in animal models. This effect has not been reported in humans.
Lithium
Theoretically, uva ursi may increase lithium levels, necessitating a decrease in dose.
Uva ursi may have diuretic properties. Diuretics may increase lithium reabsorption with sodium in the proximal tubule of the kidney. Theoretically, uva ursi might reduce excretion and increase levels of lithium.
Urinary Acidifying Agents
Effects of uva ursi in the urinary tract may be reduced by urinary acidifying agents.
Uva ursi seems to work best in alkaline urine. Theoretically, taking uva ursi with medications known to acidify the urine may decrease any effects of uva ursi on the urinary tract.
P-Glycoprotein Substrates
Theoretically, uva ursi may alter the levels of drugs transported by P-glycoprotein.
In vitro, uva ursi appears to inhibit the multi-drug transporter protein, P-glycoprotein. This effect has not been reported in humans.
