CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Acetaminophen, Butalbital, Caffeine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Uva Ursi interacts with.
In vitro, uva ursi appears to inhibit CYP3A4. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist for your headache medicine (Tecnal, Esgic Plus and similar). The butalbital piece is processed in part by a liver enzyme called CYP3A4. In lab dish studies, uva ursi seems to slow that same enzyme down. If that held true in people, your body might clear the butalbital a little more slowly, so it could build up and leave you feeling extra drowsy, lightheaded, or foggy.
I want to be honest about the strength of this: it's only been seen in test tubes, not in actual patients, so it's more of a theoretical caution than a proven problem. Still, since uva ursi is meant only for short-term use and both can be hard on the liver, it's worth a quick chat with your pharmacist or doctor before combining them.

GLUCURONIDATED DRUGS
2 of 2Acetaminophen, Butalbital, Caffeine also falls under GLUCURONIDATED DRUGS.
In vitro, uva ursi extract appears to strongly inhibit UDP-glucuronosyltransferase (UGT) 1A1 (UGT1A1). However, uva ursi extract does not appear to inhibit UGT1A1 in animal models. This effect has not been reported in humans.

Here's the gist: your headache medicine contains acetaminophen (Tylenol), and your liver clears acetaminophen partly through a process called glucuronidation. In a test-tube study, uva ursi seemed to slow one of those processing enzymes. On paper, that could let acetaminophen build up a bit, which in theory might mean more liver stress, especially if you're already taking a lot of it or using it for a while.
Now the important part: this effect only showed up in the lab, and it did not hold up in animal studies and has never been seen in people. So I wouldn't lose sleep over it. Still, uva ursi can be hard on the liver by itself, and so can acetaminophen, so it's worth a quick chat with your pharmacist or doctor before combining them.

Uva ursi is a traditional herb used mainly for urinary tract infections, and its leaves contain a compound called arbutin that may have antimicrobial effects in the urine. Evidence in people is limited, and because high doses or long-term use may be harmful to the liver and could be toxic, it should only be used short-term and with guidance from a healthcare professional. Avoid it during pregnancy and breastfeeding.

This combination product — available as capsules, tablets, and oral solution under various names including Fioricet — is used to relieve the symptoms of tension headaches (also called muscle contraction headaches).
All supported products, including Fioricet capsules and the oral solution, share this same indication. Because butalbital can be habit-forming, extended or repeated use is not recommended, and these products are not established as safe or effective for treating headaches that come back frequently over time.
