Verapamil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Uva Ursi interacts with.

Theoretically, uva ursi may decrease the metabolism of CYP3A4 substrates.
In vitro, uva ursi appears to inhibit CYP3A4. This effect has not been reported in humans.

Here's the gist: your body relies on a liver enzyme called CYP3A4 to clear verapamil out of your system. In lab tests, uva ursi appears to slow that enzyme down. If that were to happen in real life, verapamil could build up a bit more than usual, which might nudge its effects a little stronger. With verapamil, that could mean more lightheadedness, a slower heart rate, lower blood pressure, swollen ankles, or constipation.

I want to be honest about how soft this evidence is. This was only seen in a test tube, not in actual people, so it's more of a theoretical concern than a proven problem. Still, since uva ursi is meant for short-term use anyway, it's worth a quick chat with your pharmacist or doctor before pairing them, especially if you notice any new dizziness.

Theoretically, uva ursi may alter the levels of drugs transported by P-glycoprotein.
In vitro, uva ursi appears to inhibit the multi-drug transporter protein, P-glycoprotein. This effect has not been reported in humans.

Here's the gist for verapamil. Your body uses a kind of cellular bouncer called P-glycoprotein to help move certain drugs out of your cells and gut. In lab dish studies, uva ursi seems to block that bouncer a bit. If that were to happen in real life, less verapamil would get pushed out, so slightly more could stay in your system. With verapamil that might mean a bit more of its usual effects, like dizziness, lower blood pressure, a slower heartbeat, constipation, or swelling in the ankles.

The important part: this is only a theory based on test-tube work, and it has never been shown to actually happen in people. It's rated minor. Still, since verapamil affects your heart and blood pressure, it's worth a quick chat with your pharmacist or doctor before pairing them, especially since uva ursi is only meant for short-term use.

Uva ursi is a traditional herb used mainly for urinary tract infections, and its leaves contain a compound called arbutin that may have antimicrobial effects in the urine. Evidence in people is limited, and because high doses or long-term use may be harmful to the liver and could be toxic, it should only be used short-term and with guidance from a healthcare professional. Avoid it during pregnancy and breastfeeding.

About Verapamil

What Is Verapamil Used For?

Brand names: Isoptin SR, Verelan, Isoptin, Calan SR, Verelan SR, Calan, Covera HS, Verap, Veramil

Verapamil tablets and film-coated tablets are used to treat angina — including vasospastic (Prinzmetal's) angina, unstable angina, and chronic stable angina — as well as certain abnormal heart rhythms (arrhythmias) and high blood pressure (essential hypertension). For arrhythmias, regular-release tablets are used alongside digoxin to control heart rate in atrial flutter or atrial fibrillation, and to help prevent episodes of paroxysmal supraventricular tachycardia (SVT), a type of rapid heartbeat.

Extended-release and sustained-release oral forms (tablets and capsules) are indicated specifically for high blood pressure. Verapamil hydrochloride injection is used in hospital settings for rapid conversion of SVT to normal rhythm and for temporary control of fast heart rates in atrial flutter or atrial fibrillation.
