Quinidine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Uva Ursi interacts with.

Theoretically, uva ursi may decrease the metabolism of CYP3A4 substrates.
In vitro, uva ursi appears to inhibit CYP3A4. This effect has not been reported in humans.

Here's the gist for quinidine: your body relies on an enzyme called CYP3A4 to help clear it from your bloodstream. In lab dish studies, uva ursi seemed to slow that enzyme down. If that were to happen in a person, quinidine could build up a bit more than expected, which might mean stronger effects like an upset stomach, dizziness, ringing in the ears, or, more importantly, changes in your heart rhythm. Since quinidine is a heart medicine with a narrow safety window, even small shifts deserve attention.

The reassuring part: this is only a test-tube finding and has never been shown in real people, so it's theoretical. Still, please run it by your pharmacist or doctor before combining them.

Theoretically, uva ursi may alter the levels of drugs transported by P-glycoprotein.
In vitro, uva ursi appears to inhibit the multi-drug transporter protein, P-glycoprotein. This effect has not been reported in humans.

Here's the gist for quinidine: your body uses a tiny pump called P-glycoprotein to move this drug around and help clear it. In a lab dish, uva ursi seemed to block that pump. If that same thing happened in a person, quinidine could build up a bit more than expected, and since quinidine is a heart rhythm drug with a fairly narrow safe range, even small shifts can matter. More drug could mean things like nausea, ringing in the ears, dizziness, or in rare cases new rhythm problems.

That said, this is only a theoretical concern from test-tube work, never actually shown in people, so I wouldn't lose sleep over it. Still, because quinidine is one to handle carefully, check with your pharmacist or doctor before adding uva ursi.

Uva ursi is a traditional herb used mainly for urinary tract infections, and its leaves contain a compound called arbutin that may have antimicrobial effects in the urine. Evidence in people is limited, and because high doses or long-term use may be harmful to the liver and could be toxic, it should only be used short-term and with guidance from a healthcare professional. Avoid it during pregnancy and breastfeeding.

About Quinidine

What Is Quinidine Used For?

Brand names: Quinaglute Duratabs, Quinaglute, Duraquin, Cardioquin, Cin-Quin, Quinora, Lin-Qin, Quinova, Quinidex Extentabs

Quinidine is used to help restore a normal heart rhythm in people with atrial fibrillation or atrial flutter (irregular, rapid heart rhythms originating in the upper chambers). It is also used on an ongoing basis to reduce how often those episodes come back, in patients whose episodes are frequent and poorly tolerated — but only after other options have been tried first.

Quinidine sulfate oral tablets and quinidine gluconate extended-release oral tablets are both approved for these heart-rhythm uses. Quinidine sulfate tablets are additionally used as part of an approved treatment regimen for life-threatening Plasmodium falciparum malaria, in combination with quinidine gluconate injection (the central component of that regimen). Both formulations may also be used to suppress recurrent, documented life-threatening ventricular arrhythmias (dangerous rapid rhythms in the lower chambers of the heart), such as sustained ventricular tachycardia.
