Terfenadine falls under P-GLYCOPROTEIN SUBSTRATES — the group Uva Ursi interacts with.

Theoretically, uva ursi may alter the levels of drugs transported by P-glycoprotein.
In vitro, uva ursi appears to inhibit the multi-drug transporter protein, P-glycoprotein. This effect has not been reported in humans.

Here's the gist for these two together. Uva ursi has shown, in lab dish studies only, that it can block a transporter called P-glycoprotein, which is one of the pumps that helps move terfenadine out of your system. If that pump slows down, terfenadine levels could in theory creep up.

That matters because terfenadine isn't a typical allergy pill anymore. It was pulled from the market because higher blood levels can disturb the heart's rhythm (a problem called QT prolongation) and cause dangerous, irregular heartbeats. That said, this concern comes purely from test-tube work, not from people, so it's theoretical. Still, given terfenadine's heart risks, please check with your pharmacist or doctor before combining them.
