Diltiazem falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Uva Ursi interacts with.

Theoretically, uva ursi may decrease the metabolism of CYP3A4 substrates.
In vitro, uva ursi appears to inhibit CYP3A4. This effect has not been reported in humans.

Here's the gist for diltiazem (Cardizem and its long-acting versions). Your body relies on a liver enzyme called CYP3A4 to clear diltiazem out of your system. In lab dish studies, uva ursi seems to slow that same enzyme down. If that held true in people, your body could clear diltiazem a little more slowly, letting more of it build up.

What might that feel like? Possibly more of diltiazem's usual effects, like lower blood pressure, dizziness, a slower heartbeat, or swelling in the ankles. I want to be honest though: this is only a theoretical concern from test-tube work, never actually shown in humans. I wouldn't lose sleep over it, but since you're on a heart medicine, run it by your pharmacist or doctor before adding uva ursi.

Theoretically, uva ursi may alter the levels of drugs transported by P-glycoprotein.
In vitro, uva ursi appears to inhibit the multi-drug transporter protein, P-glycoprotein. This effect has not been reported in humans.

Here's the gist for diltiazem (Cardizem and similar): your body uses a little cellular pump called P-glycoprotein to help move certain drugs around and out. In lab dish studies, uva ursi seemed to slow that pump down. If that were to happen in people, slightly more diltiazem could stay in your system, which in theory might nudge its effects up a bit, think lower blood pressure, dizziness, a slower heart rate, or some ankle swelling.

The honest part: this is only a theoretical, test-tube finding and hasn't been seen in actual people, so the real-world risk looks small. Still, since uva ursi is meant for short-term use only, it's worth a quick word with your pharmacist or doctor before pairing the two.

Uva ursi is a traditional herb used mainly for urinary tract infections, and its leaves contain a compound called arbutin that may have antimicrobial effects in the urine. Evidence in people is limited, and because high doses or long-term use may be harmful to the liver and could be toxic, it should only be used short-term and with guidance from a healthcare professional. Avoid it during pregnancy and breastfeeding.

What Is Diltiazem Used For?

Brand names: Cardizem, Cardizem SR, Diltiaz SR, Cardizem CD, Tiazac, Diltiaz XT, Diltiaz CD, Dilacor, Diltiaz, Cardizem LA, Dilacor XR, Cartia XT, Tiamate, Adizem-SR, Adizem-XL, Angitil SR, Angitil XL, Calcicard CR, Dilcardia SR, Dilzem SR, Dilzem XL, Slozem, Tildiem LA, Tildiem Retard, Viazem XL, Zemtard 120XL, Zemtard 180XL, Zemtard 240XL, Zemtard 300XL

Oral extended-release forms of diltiazem — including Matzim LA, Cardizem LA, Cartia, Cartia XT, Tiadylt ER, and various generic capsules and tablets — are used to treat high blood pressure (hypertension) and chronic stable angina (chest pain brought on by activity). Some capsule formulations, including Cartia, Cartia XT, and certain generics, are also approved for angina due to coronary artery spasm (sudden chest pain caused by a sudden tightening of a coronary artery).

Injectable forms of diltiazem are used in hospital settings to temporarily slow a dangerously fast heart rate in atrial fibrillation or atrial flutter (rapid, irregular heart rhythms), and to restore a normal heart rhythm in paroxysmal supraventricular tachycardia (PSVT) — sudden episodes of a very fast but regular heartbeat originating above the heart's lower chambers.
