Why this applies to Alvimopan
Alvimopan falls under P-GLYCOPROTEIN SUBSTRATES — the group Uva Ursi interacts with.
In vitro, uva ursi appears to inhibit the multi-drug transporter protein, P-glycoprotein. This effect has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the short version: alvimopan (Entereg) is moved around the body partly by a transporter called P-glycoprotein, which acts like a gatekeeper at the gut wall. In lab dish studies, uva ursi seemed to block that gatekeeper. If that happened in a person, a bit more alvimopan could get absorbed than usual. With this drug, that mostly means a slightly higher chance of side effects like indigestion or stomach upset.
The reassuring part: this is only a theory based on test-tube data, and it has never actually been seen in people. The interaction is rated minor. Still, alvimopan is given in a hospital after bowel surgery for a short time, so it's worth mentioning any herbs to your surgical team or pharmacist.

Uva ursi is a traditional herb used mainly for urinary tract infections, and its leaves contain a compound called arbutin that may have antimicrobial effects in the urine. Evidence in people is limited, and because high doses or long-term use may be harmful to the liver and could be toxic, it should only be used short-term and with guidance from a healthcare professional. Avoid it during pregnancy and breastfeeding.

What Is Alvimopan Used For?
Brand name: Entereg
Alvimopan capsules are used in hospitals to help the digestive tract recover more quickly after surgery that involves removing part of the bowel (partial bowel resection with primary anastomosis — meaning the remaining ends are reconnected). After this type of surgery, the bowel often temporarily stops working, a condition called postoperative ileus.
This medication is only given during a short hospital stay and is not for long-term or home use.
