Water Balance Ingredients & Drug Interactions
What is this page for?
First and foremost: checking Water Balance against your medications. The heart of this page is the interaction checker and the full interaction report — how this product’s ingredients may interact with prescription and over-the-counter medicines you may be taking.
Around that, we add a pharmacist’s high-level view of the product as a whole — what’s inside, the evidence for its stated use, how transparent the label is, and what safety data exists — so you can see the full picture in one place. It’s educational information from our licensed clinical databases and the clinical staff at HelloPharmacist — not medical advice — and we don’t sell or endorse products. Our editorial policy

What’s inside
Low disclosure
Water Balance contains 15 active and inactive ingredients across three blended components (Proprietary Blend, AdPT, and EDS) plus individual herbs and enzymes. The key active ingredients include ashwagandha (a traditional adaptogenic herb), digestive enzymes (protease, lipase, and cellulase for breaking down food), medicinal mushrooms (cordyceps and reishi), plant extracts (burdock, dandelion, juniper, rhodiola, gynostemma, eleuthero, birch, and horsetail), and amylase.
The inactive ingredients are cellulose and water.
Does it work?
Couldn't assess
The evidence for most of these ingredients is sparse. Ashwagandha is possibly effective for insomnia, anxiety, stress, and generalized anxiety disorder, but the safety data are limited and long-term effects are not well studied.
Gynostemma (jiaogulan) is possibly effective for diabetes. Eleuthero is possibly effective for genital herpes.
The remaining ingredients — burdock, dandelion, juniper, rhodiola, cordyceps, reishi mushroom, birch, and horsetail, plus the digestive enzymes — all show insufficient evidence to rate their effectiveness for any condition. Cordyceps is actually listed as possibly ineffective for athletic performance, and reishi as possibly ineffective for high cholesterol.
How safe is it?
Well-documented data
Ashwagandha is generally well tolerated short-term but raises concerns: it's traditionally avoided in pregnancy due to miscarriage risk, and safety while breastfeeding is not established — the facts advise against both. The most serious rare adverse effect is liver damage (acute hepatitis, liver failure), though diarrhea, nausea, and stomach upset are more common.
A case report links it to very high thyroid hormone levels after 2 years of use. The digestive enzymes (protease, lipase) are generally well tolerated but may cause digestive upset or allergic reactions; protease safety in pregnancy is not established, and lipase safety while breastfeeding is not well studied.
Burdock, dandelion, juniper, rhodiola, cordyceps, reishi, eleuthero, and gynostemma all advise against use in pregnancy and breastfeeding due to insufficient safety data. Most are generally well tolerated short-term, though dandelion may cause diarrhea and heartburn, juniper may irritate kidneys with long-term use, and horsetail contains thiaminase (which can cause thiamine deficiency with prolonged use) — long-term safety is not well studied for any of these.
Meds to double-check
Moderate interaction found
Before taking Water Balance, double-check if you use any blood thinners or antiplatelet drugs (aspirin, warfarin, clopidogrel) — ashwagandha, burdock, dandelion, cordyceps, eleuthero, gynostemma, and reishi all interact at Moderate severity. Next, check blood pressure medications (especially losartan), diabetes drugs, immunosuppressants, lithium, digoxin, thyroid hormone, diuretics, liver-damaging drugs, antidepressants, anti-anxiety drugs, and certain antibiotics (fluoroquinolones).
Many of these interactions are Moderate severity and could affect how your medications work. No interactions are documented for the ingredients we could not check.

Water Balance by Professional Botanicals Drug Interactions
HelloPharmacist Interaction Report
Water Balance by Professional Botanicals contains 15 ingredients, several of which interact with medications.
The most serious interaction involves ashwagandha, which can theoretically increase the risk of liver damage (hepatotoxicity) when taken with drugs that are toxic to the liver — a Moderate severity concern. This same ingredient also raises Moderate-severity risks with CNS depressants (sedatives, anti-anxiety drugs, and sleep aids), blood pressure medications, benzodiazepines, diabetes medications, thyroid hormone, and drugs metabolized by a liver enzyme called CYP1A2.
Read the full breakdown — every affected drug type, severity by severity
Dandelion, rhodiola, eleuthero, and cordyceps all interact with blood thinners (anticoagulants and antiplatelet drugs) at Moderate severity. Dandelion additionally affects potassium-sparing water pills, blood sugar drugs, certain antibiotics (fluoroquinolones), lithium, and a group of medications processed by UDP-glucuronosyltransferase.
Rhodiola interacts with blood pressure medications, diabetes drugs, and several liver enzymes (CYP2C9, CYP1A2, CYP3A4, and P-glycoprotein substrates) at Moderate severity, plus a specific blood pressure drug, losartan. Eleuthero affects similar enzyme systems, plus antidiabetes drugs, heart medications (digoxin), and immunosuppressants.
Cordyceps and gynostemma (jiaogulan) also interact with immunosuppressants and diabetes medications.
Juniper has Minor-severity interactions with diuretics and lithium, and Moderate interactions with diabetes drugs. Burdock, reishi mushroom, horsetail, and birch round out the list with effects on blood thinners, blood pressure pills, diabetes drugs, diuretics, and lithium.
Altogether, these interactions span 1,510 individual medications.

For each ingredient in Water Balance with known interactions, here are the types of medications they can affect. Open any type for the detail — or search your exact drug in the checker above.
Dandelion
Anticoagulant/Antiplatelet Drugs
Theoretically, taking dandelion root along with anticoagulant or antiplatelet drugs might increase the risk of bruising and bleeding.
In vitro research suggests that dandelion root inhibits platelet aggregation.
Antidiabetes Drugs
Theoretically, dandelion might increase the risk for hypoglycemia when used with antidiabetes drugs.
Laboratory research suggests that dandelion extract may have moderate alpha-glucosidase inhibitor activity and might also increase insulin secretion. Also, in a case report, a 58-year-old woman with type 2 diabetes who was being treated with insulin developed hypoglycemia 2 weeks after beginning to eat salads containing dandelion.
Cytochrome P450 1A2 (Cyp1A2) Substrates
Theoretically, dandelion might increase levels of drugs metabolized by CYP1A2.
Laboratory research suggests that dandelion might inhibit CYP1A2. So far, this interaction has not been reported in humans. However, until more is known, watch for an increase in the levels of drugs metabolized by CYP1A2 in patients taking dandelion.
Glucuronidated Drugs
Theoretically, dandelion might increase the clearance of drugs that are UDP-glucuronosyltransferase substrates.
There is some preliminary evidence that dandelion might induce UDP-glucuronosyltransferase, a phase II enzyme.
Lithium
Theoretically, through diuretic effects, dandelion might reduce excretion and increase levels of lithium.
Animal research suggests that dandelion has diuretic properties. As diuretics can increase serum lithium levels, the dose of lithium might need to be decreased when taken with dandelion.
Potassium-Sparing Diuretics
Theoretically, dandelion might increase the risk of hyperkalemia when taken with potassium-sparing diuretics.
Dandelion contains significant amounts of potassium.
Quinolone Antibiotics
Theoretically, dandelion might lower fluoroquinolone levels.
Animal research shows that dandelion reduces absorption of ciprofloxacin and can lower levels by 73%. However, this effect has not been reported in humans.
Horsetail
Antidiabetes Drugs
Theoretically, taking horsetail with antidiabetes drugs might increase the risk of hypoglycemia.
Equisetum myriochaetum has demonstrated hypoglycemic activity in clinical research. In an animal diabetic model, Equisetum giganteum had hypoglycemic effects. It is unclear whether other horsetail species have hypoglycemic effects.
Diuretic Drugs
Theoretically, taking horsetail with diuretic drugs might increase potassium loss and the risk of hypokalemia.
Laboratory research shows that various species of horsetail have diuretic properties. Due to its diuretic effects, there has been concern that taking horsetail along with potassium-depleting diuretics might increase the risk for hypokalemia. However, pharmacokinetic research in humans shows that taking horsetail 900 mg daily for 4 days does not affect urinary excretion of electrolytes, including potassium and sodium, despite having a diuretic effect similar to taking hydrochlorothiazide 25 mg daily. It is unclear if taking horsetail for a longer duration would affect electrolyte levels. Until more is known, use with caution.
Efavirenz (Sustiva)
Theoretically, horsetail might decrease the levels and clinical effects of efavirenz.
In two case reports, patients were found to have detectable viral loads when taking horsetail-containing supplements along with an antiretroviral regimen that included efavirenz. In one case, the antiretroviral regimen included zidovudine, lamivudine, and efavirenz; in the other case, the regimen consisted of emtricitabine, tenofovir disoproxil fumarate, and efavirenz. One month after discontinuing horsetail, the viral loads became undetectable in both cases. The exact mechanism of this interaction is unknown. It is also unclear if this interaction is specific to efavirenz or if it is related to various components of antiretroviral therapy.
Lithium
Theoretically, horsetail might increase the levels and adverse effects of lithium.
Animal research suggests that horsetail has diuretic properties. Theoretically, due to these potential diuretic effects, horsetail might reduce excretion and increase levels of lithium. The dose of lithium might need to be decreased.
Nucleoside Reverse Transcriptase Inhibitors (Nrtis)
Theoretically, horsetail might decrease the levels and clinical effects of NRTIs.
In two case reports, patients were found to have detectable viral loads when taking horsetail-containing supplements along with an antiretroviral therapy. In one case, the antiretroviral regimen included zidovudine, lamivudine, and efavirenz; in the other case, the regimen consisted of emtricitabine, tenofovir disoproxil fumarate, and efavirenz. One month after discontinuing the supplement, the viral loads became undetectable in both cases. The exact mechanism of these interactions is unknown. It is also unclear if these interactions are specific to NRTIs or if they are related to various components of antiretroviral therapy.
Juniper
Antidiabetes Drugs
Theoretically, taking juniper berry with antidiabetes medications might cause additive hypoglycemia.
Animal research shows that juniper berry can lower blood glucose.
Diuretic Drugs
Theoretically, juniper berry might increase the risk of adverse effects from diuretic drugs.
Juniper berry is thought to have mild diuretic effects.
Lithium
Theoretically, juniper berry might reduce lithium excretion and increase serum levels of lithium.
Juniper berry is thought to have mild diuretic effects.
Burdock
Anticoagulant/Antiplatelet Drugs
Theoretically, taking burdock with anticoagulant or antiplatelet drugs might increase the risk of bleeding.
In vitro research shows that lignans from burdock reduce rabbit platelet aggregation by inhibiting platelet activating factor. This interaction has not been reported in humans.
Birch
Diuretic Drugs
Theoretically, birch might interfere with diuretic therapy.
