Acetaminophen, Dextromethorphan, Guaifenesin, Phenylephrine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Rhodiola interacts with.

Theoretically, rhodiola might increase levels of drugs metabolized by CYP1A2.
In vitro research shows that rhodiola inhibits CYP1A2. This effect is highly variable and appears to be dependent on the rhodiola product studied. However, a clinical study in healthy young males found that taking rhodiola extract 290 mg daily for 14 days does not inhibit the metabolism of caffeine, a CYP1A2 substrate.

Here's the gist for this cold and cough combo (Conar-A). On paper, rhodiola can slow down a liver enzyme called CYP1A2, and that's the pathway involved here. If that enzyme runs slower, a medicine that uses it could build up a bit more than usual. Of these four ingredients, none is a prodrug that needs activating, so the theoretical concern would be slightly higher levels rather than a weaker effect.

Honestly though, I wouldn't lose sleep over this one. The evidence is weak and mostly from lab dishes, and a human study found rhodiola didn't actually slow this enzyme in real life. At most you might notice a touch more drowsiness or jitteriness. Still, run it by your pharmacist or doctor first, especially with the phenylephrine.

This is a multi-symptom cold and cough combo, and the piece that matters here is the dextromethorphan, since it's partly handled by the CYP3A4 enzyme. In a test tube, rhodiola can slow that enzyme down, which in theory could let dextromethorphan build up a bit and lead to more drowsiness, dizziness, or feeling foggy. The acetaminophen, guaifenesin, and phenylephrine aren't really affected by this pathway.

Here's the reassuring part: this is rated a minor interaction, and a real human study found that rhodiola didn't actually change how the body cleared a similar CYP3A4 drug. So for most people taking this for a short cold, the risk is low. Still, since these products vary a lot, it's worth a quick word with your pharmacist before combining them.
