Ashwagandha has been linked to cases of acute hepatitis, liver failure, hepatic encephalopathy, autoimmune hepatitis, the need for liver transplantation, and death due to liver failure.

This cold and flu combo contains acetaminophen (the same active ingredient as Tylenol), which your liver has to process. When taken in large amounts or over many days, acetaminophen itself can be hard on the liver. Ashwagandha has been tied, in rare reports, to liver injury too. So the concern here is simple: two things that can each stress your liver, used together, might add up.
To be fair, this is a theoretical caution based on how these work, not proof it will happen, and occasional, correctly-dosed use is far less worrisome. Still, watch for warning signs like yellowing skin or eyes, dark urine, belly pain, or unusual tiredness. Avoid alcohol, never exceed the acetaminophen label dose, and check with your pharmacist before combining these.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

In vitro research shows that ashwagandha extract induces CYP1A2 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with this Coricidin HBP combo product (which packs acetaminophen for pain and fever, chlorpheniramine for runny nose, and dextromethorphan for cough). Lab and animal studies hint that ashwagandha may gently speed up an enzyme called CYP1A2. In theory, a faster enzyme could clear these medicines a touch quicker, which might make them feel slightly less effective. So you'd notice a bit more cough, congestion, or aches breaking through, not anything dangerous.
Honestly, this is a very weak, theoretical concern based only on test-tube and animal data, and the cold medicine itself isn't really cleared by CYP1A2 in a major way. For short-term cold use I wouldn't lose sleep over it. Still, it never hurts to run it by your pharmacist, especially if you take ashwagandha daily.

This Coricidin product is a three-in-one cold and flu combo: acetaminophen for fever and aches, chlorpheniramine (an antihistamine) for runny nose, and dextromethorphan to quiet a cough. One of these, dextromethorphan, is partly handled by a liver enzyme called CYP3A4. In lab dish studies only, ashwagandha seemed to speed up that enzyme, which could in theory clear the drug a little faster and make the cough relief slightly weaker.
Honestly, this is a very minor, theoretical concern based on test-tube data, not real people. For an occasional cold remedy taken for a few days, it's unlikely to cause any noticeable problem. If you take ashwagandha regularly and want reassurance, just run it by your pharmacist.

Serotonergic Drugs

The piece in this combo cold product that matters here is the dextromethorphan (the cough suppressant). It nudges serotonin levels in the brain, and ashwagandha may gently affect serotonin pathways too. In theory, stacking them could add up. In practice, though, this is a minor concern. The evidence comes mostly from animal studies, and there are no real-world reports of problems when ashwagandha is paired with serotonin-affecting medicines.
So I wouldn't lose sleep over an occasional dose of this Coricidin product with ashwagandha. Just be aware of signs like feeling unusually jittery, sweaty, fast heartbeat, shivering, or confusion, which would be reasons to stop and get checked. The chlorpheniramine can also make you drowsy, and ashwagandha may add to that calming effect. If you take either regularly, run it by your pharmacist first.
