Cyanocobalamin is a prescription vitamin B12 used to treat or prevent B12 deficiency across several product types, routes, and brands. Uses vary by product:

Inside the body, cyanocobalamin is converted into an active coenzyme form of vitamin B12. This coenzyme is essential for two key reactions: converting a compound called methylmalonate into succinate (important for energy metabolism), and helping produce methionine from homocysteine — a reaction that also requires folate. Without enough active B12, folate gets locked in an inactive storage form and can't support healthy red blood cell production, which leads to a type of anemia called megaloblastic anemia. B12 also helps maintain certain enzyme systems involved in fat, carbohydrate, and protein metabolism.

- Leber's disease: Patients with this hereditary optic nerve condition must not use cyanocobalamin — it has caused rapid, severe vision loss. A different B12 form (such as hydroxocobalamin) should be used instead.
- Severe allergic reactions: Anaphylactic shock and death have been reported after injectable B12. A test dose may be given before starting nasal spray in patients who have never tolerated injectable cyanocobalamin.
- Low potassium and sudden death: Intensively treating severe megaloblastic anemia with B12 can cause potassium levels to drop dangerously. Potassium and platelet counts must be monitored during therapy.
- Masking folate deficiency: High B12 doses can improve blood counts even when the real problem is folate deficiency, hiding a diagnosis that still needs treatment. Both B12 and folate levels should be checked before starting therapy.
- Polycythemia vera: B12 deficiency can suppress signs of this blood disorder. Starting B12 therapy may unmask it — patients showing signs should be evaluated further.
- Benzyl alcohol and aluminum in injectables: Some injectable products contain benzyl alcohol (linked to fatal reactions in premature infants) or aluminum (which can accumulate to toxic levels in patients with impaired kidney function, especially premature newborns).
