How we grade severity & evidence
Severity levels
- Contraindicated: These should generally not be used together.
- Major: Potentially serious — often needs a change or close monitoring.
- Moderate: Can be significant — usually manageable with monitoring.
- Minor: Usually limited clinical impact.
Evidence grades
- Established: Well documented — supported by controlled studies or strong clinical data.
- Probable: Good supporting evidence, though not definitively proven.
- Suspected: Some evidence suggests this interaction, but it is not well established.
- Possible: Limited or conflicting evidence; the interaction may occur.
- Theoretical: Predicted from the drugs' pharmacology; not yet confirmed in people.
Ratings come from the documented interaction literature and are reviewed by a pharmacist. They describe the documented risk of the combination, not what will necessarily happen to you — your dose, timing, and health picture all matter.
What happens
Increased buPROPion exposure, reduced hydroxybuPROPion exposure
Interaction Deep Dive
Coadministration of buPROPion with clopidogrel (CYP2B6 inhibitor) is not recommended. Concomitant use of these drugs may increase buPROPion exposures but may reduce hydroxybuPROPion exposure. Based on clinical response, dosage adjustment of buPROPion is necessary. In a drug interaction study in healthy male volunteers, clopidogrel 75 mg once daily or ticlopidine 250 mg twice daily increased exposure of buPROPion by 40 % and 60% for clopidogrel, by 38% and 85% for ticlopidine, respectively. The exposure of hydroxybuPROPion were decreased1234 50% and 52%, respectively, by clopidogrel, and 78% and 84%, respectively, by ticlopidine. This effect is thought to be due to the inhibition of the CYP2B6-catalyzed buPROPion hydroxylation 23.
Why it happens (mechanism)
Inhibition of CYP2B6-mediated metabolism of buPROPion
