Increased digoxin exposure and an increased risk of digoxin toxicity

Interaction Deep Dive

Concomitant use of digoxin (a P-gp substrate with a narrow therapeutic index) and flibanserin (P-gp inhibitor) may increase digoxin exposure and result in digoxin toxicity. If these agents are coadministered, increase monitoring frequency of digoxin exposure3, and monitor for potential signs and symptoms of clinical toxicity when initiating, adjusting, or discontinuing flibanserin 12. In a drug interaction study, administration of flibanserin 100 mg once daily for 5 days followed by a single dose of digoxin 0.5 mg in 24 healthy volunteers increased digoxin AUC(0 to infinity) by 2 fold and Cmax by 1.5 fold compared with digoxin administered alone 3.
