Here is the interaction we found between these two, with the evidence behind it.
CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES
Why this applies to Acetaminophen, Diphenhydramine
Acetaminophen, Diphenhydramine falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group African Wild Potato interacts with.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with Tylenol PM, which combines acetaminophen with the antihistamine diphenhydramine. In lab dish studies (not in people), African Wild Potato seems to slow down a liver enzyme called CYP1A2. If that same slowdown happened in your body, drugs cleared by that enzyme could linger a bit longer and hit harder. For this combo, that might mean stronger drowsiness, more dry mouth, dizziness, blurry vision, or grogginess the next morning from the diphenhydramine side.
The honest part: this is only test-tube evidence, so it's theoretical and hasn't been shown in real patients. I wouldn't lose sleep over it, but since occasional use of a sleep aid plus a supplement is easy to overdo, run it by your pharmacist or doctor first.
