Acetaminophen, Aspirin, Caffeine falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Allspice interacts with.

Theoretically, combining allspice with an antiplatelet or anticoagulant drug might increase the risk of bleeding.
Eugenol, a constituent of allspice, is reported to have antiplatelet activity. However, this interaction has yet not been reported in humans.

Here's the thing to know about products like Excedrin: one of its ingredients, aspirin, already thins your blood a bit by making platelets less sticky. Allspice contains eugenol, which lab studies suggest may have its own mild platelet-slowing effect. So on paper, piling concentrated allspice on top of aspirin could add up and slightly raise your chance of bleeding or bruising. Think easy bruising, nosebleeds, or a cut that's slow to stop.

That said, this is purely theoretical and hasn't actually been seen in people. The pinch of allspice you cook with is very unlikely to matter. Concentrated extracts or the essential oil are where I'd be more cautious. If you use those regularly, just run it by your pharmacist or doctor first.
