Ezetimibe, Atorvastatin falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group American Elder interacts with.

Theoretically, taking American elder might increase levels of CYP3A4 substrates.

Your combo pill pairs ezetimibe with atorvastatin, and the atorvastatin part is the piece that matters here. Atorvastatin is cleared by a liver enzyme called CYP3A4. In test tube studies, American elder appears to slow that enzyme down. If that happened in your body, atorvastatin could build up a bit higher than usual, which can raise the odds of muscle aches, weakness, or rarely more serious muscle problems.

That said, this is only a theoretical concern from lab data, not from real people, so I wouldn't lose sleep over it. A little cooked elderberry in food is very unlikely to matter. If you take a concentrated elderberry supplement daily, just mention it to your pharmacist or doctor so they can keep an eye on things.
