Here's something worth knowing if you're taking Ibsrela (tenapanor) for IBS-C and thinking about elderberry. Lab studies hint that American elder may slow down an enzyme called CYP3A4, which the body uses to process many drugs. In theory, that could let a CYP3A4 medication build up a bit and cause stronger effects.

The good news for tenapanor specifically: it works mostly right in your gut and very little of it actually gets absorbed into your bloodstream, so this enzyme issue is unlikely to matter much in real life. The main thing to watch for would be more diarrhea, cramping, or bloating. Also remember this is only test-tube evidence, not proven in people. Food amounts of cooked berries are unlikely to be a problem, but check with your pharmacist before using concentrated elderberry supplements.

Tenapanor is approved for two distinct conditions, each under a separate brand. Ibsrela is used to treat irritable bowel syndrome with constipation (IBS-C) in adults. XPHOZAH is used to reduce high phosphorus levels (hyperphosphatemia) in adults with chronic kidney disease (CKD) who are on dialysis — specifically as an add-on when phosphate binders haven't worked well enough or aren't tolerated.
