Tenapanor Hydrochloride falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Acacia Rigidula interacts with.

Theoretically, Acacia rigidula products that are adulterated with beta-methylphenylethylamine (BMPEA) might increase the levels of CYP3A4 substrates.
In vitro research shows that BMPEA, an adulterant found in many Acacia rigidula products, inhibits CYP3A4 enzyme by 37%.

Here's the gist for Ibsrela (tenapanor). Some Acacia rigidula products have been found spiked with a hidden stimulant called BMPEA, and lab tests suggest BMPEA can partly block the CYP3A4 enzyme. In theory, blocking that enzyme could let drugs cleared by it build up a bit. Tenapanor isn't a prodrug, so more drug in your system would mean stronger effects.

The good news is that tenapanor stays mostly in your gut and very little is absorbed, so this interaction is largely theoretical and based only on test-tube data. Still, the main thing you'd notice from extra tenapanor effect is worse diarrhea, cramping, or bloating. Honestly, the bigger concern is the unapproved stimulant itself. I'd skip this product and check with your pharmacist first.

Tenapanor is approved for two distinct conditions, each under a separate brand. Ibsrela is used to treat irritable bowel syndrome with constipation (IBS-C) in adults. XPHOZAH is used to reduce high phosphorus levels (hyperphosphatemia) in adults with chronic kidney disease (CKD) who are on dialysis — specifically as an add-on when phosphate binders haven't worked well enough or aren't tolerated.
