Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

The medication you're asking about includes methyltestosterone, which is itself an androgen (a male-type hormone). Andarine (S-4) is a SARM that acts on those very same androgen receptors. So instead of two different mechanisms clashing, you'd basically be stacking one androgen on top of another. That adds up, and the worry is simply more androgen activity than your body would get from the prescription alone.

In real terms, that could mean more acne, oily skin, hair loss, mood swings or irritability, and a higher chance of liver strain, plus masculinizing effects that hit especially hard for women. Keep in mind this is a theoretical, lower-evidence concern, but Andarine is also illegal and unstudied in people. Please loop in your doctor or pharmacist before combining anything here.

From our pharmacist’s overview on the Diethylstilbestrol, Methyltestosterone page

“Tylosterone pairs diethylstilbestrol, an estrogen, with methyltestosterone, an oral androgen already known for stressing the liver, and that is exactly where this interaction list concentrates. I would handle these 73 supplements carefully.”

Andarine

  • Strength gains

Andarine (S-4) is a synthetic selective androgen receptor modulator (SARM) that is not approved by the FDA for human use and is banned in sports. It is not a legal dietary supplement ingredient, its long-term safety in people is unknown, and reported side effects include vision changes and possible liver and hormone problems. Because of these risks and its illegal status, we do not recommend using it.
