Theoretically, anise oil might increase the effects and adverse effects of codeine.
Animal research shows that anise oil increases the analgesic effects of codeine, possibly by inducing its phase I metabolism and increasing conversion to morphine. Whether this interaction occurs in humans is unclear.

Here's the key thing about codeine: it's a prodrug. By itself it does very little. Your body has to convert it into morphine to actually relieve pain. So anything that speeds up that conversion can make codeine hit harder.

The concern with anise is exactly that. In animal studies, anise oil appeared to ramp up the enzyme work that turns codeine into morphine, which boosted its pain-relieving effect. The flip side of a stronger effect is stronger side effects, things like more drowsiness, constipation, nausea, and in larger amounts slowed breathing. I'll be honest with you though: this is based only on lab and animal data, and we don't know if it happens in people. A little anise tea or seasoning is very unlikely to matter; concentrated anise oil is the bigger question mark. If you use that, check with your pharmacist or doctor first.
