Brexpiprazole falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Astaxanthin interacts with.

Theoretically, astaxanthin may decrease levels of drugs metabolized by CYP3A4.
In vitro research shows that astaxanthin induces CYP3A4 enzyme activity in human hepatocytes. This effect has not been reported in humans.

Here's the gist with Rexulti (brexpiprazole) and astaxanthin. Your body clears brexpiprazole mainly through a liver enzyme called CYP3A4. In a lab dish, astaxanthin seemed to speed that enzyme up, which in theory could clear brexpiprazole a little faster and make it slightly less effective. Brexpiprazole is the active drug itself (not a prodrug), so faster breakdown would mean less benefit, not more side effects.

The important part: this was only seen in test-tube and cell studies, never in actual people, so it may not matter at all in real life. Still, if you ever notice your mood or thinking symptoms creeping back, mention the astaxanthin to your pharmacist or prescriber before changing anything.

Astaxanthin is a reddish carotenoid pigment with strong antioxidant activity in the lab, and it is widely promoted for skin, eye, heart, and exercise benefits. Early human studies are promising for a few uses, but the overall evidence is still limited and not strong enough to make firm claims. What Is Brexpiprazole Used For?

Brand name: Rexulti

Brexpiprazole tablets are approved for two uses in adults: as an add-on (adjunctive) treatment alongside antidepressants for major depressive disorder (MDD), and as a treatment for schizophrenia. These uses apply to both the generic brexpiprazole tablets and Rexulti.

Rexulti has one additional approved use: treating agitation associated with dementia due to Alzheimer's disease in adults. Rexulti is also approved for schizophrenia in adolescents ages 13 and older. Note that Rexulti is not approved for dementia-related psychosis without agitation from Alzheimer's disease, and it is not meant to be used on an as-needed basis for Alzheimer's-related agitation.
