Lesinurad, Allopurinol falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Berberine interacts with.

Theoretically, berberine might increase serum levels of drugs metabolized by CYP2C9.
Preliminary clinical research shows that berberine can inhibit CYP2C9. Theoretically, taking berberine with drugs metabolized by CYP2C9 might increase drug levels and increase the risk of adverse effects.

Here's the deal with Duzallo, which combines lesinurad and allopurinol for gout. The lesinurad portion is partly broken down by a liver enzyme called CYP2C9. Berberine appears to slow that same enzyme down a bit, so in theory it could let lesinurad build up a little higher than usual. That might mean a greater chance of side effects like kidney strain, changes in kidney lab numbers, or headache.

I want to be honest though: this is mostly a theoretical concern backed by early, lower-quality research, not strong proof. Still, since lesinurad can be hard on the kidneys, it's worth a quick chat with your pharmacist or doctor before adding berberine. Don't stop or change anything on your own.
