Why this applies to Mavacamten
Mavacamten falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Berberine interacts with.
In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, use of berberine with drugs metabolized by CYP3A4 might increase drug levels and increase the risk of adverse effects.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the thing with Camzyos (mavacamten): your body relies on a liver enzyme called CYP3A4 to clear it out. Berberine appears to slow that same enzyme down. So pairing them could let mavacamten build up higher than expected, which matters because this medication already needs careful dose monitoring and regular heart ultrasounds.
If levels climb too high, you may see more of mavacamten's known effects, mainly a heart that doesn't squeeze as strongly (reduced ejection fraction), along with dizziness, shortness of breath, or swelling. The evidence here is mostly theoretical and based on lab and early studies, so I don't want you to panic. But because this is a sensitive drug, please talk with your cardiologist or pharmacist before adding berberine rather than starting it on your own.

It blocks CYP3A4, the liver enzyme that clears mavacamten, and too much of this drug can weaken the heart's pumping strength, the very problem careful dosing exists to prevent.”
